Disability Alliance BC has prepared this sheet to help you complete the Indigenous Services Canada (ISC) disability designation application for the Persons with Disabilities (PWD) benefit. It provides you with easy-to-follow directions that take you through the application form step by step. It includes letters to give to your doctor/nurse practitioner and assessor (a professional who can describe the impact of your disability on your daily life). There is also a checklist to help you identify the daily living activities you have trouble performing because of your disability.
